A limited number of 15454-OPT-AMP-C Units might generate an Equipment (EQPT) FAIL alarm due to a lack of communication with the Optical Module sometime after they are installed.
Cards that were built between November 20, 2012 and April 30, 2013 might be equipped with Optical Modules that can stop the communication towards the main card. Cards potentially affected by the problem are referenced in the How to Identify Affected Products section of this field notice.
The unit raises the EQPT FAIL alarm after it is inserted into the chassis.

Cards that are affected are listed in the Serial Number Validation Tool. Enter the Serial Number of the unit in order to check if it is impacted by the problem.
The Serial Number of the AMP-C Unit is available in the Inventory Tab of the Cisco Transport Controller (CTC).
