A limited quantity of V02 GLC-SX-MM-RGD units shipped between late February to early September 2016 and a limited quantity of V02 GLC-ZX-SM-RGD units shipped between late April to September 2016 with serial numbers that begin with 'ACW' might have an incompatibility issue if used in CGS 2520, IE 3010, CGR-2010 GRWIC (CGS), IE2000, or IE2000U switches.
When the issue occurs, the SFP appears inoperative and random host I2C error messages might be seen or high CPU usage until the module is removed or system power-cycled which temporarily resolves the problem.
Cisco is aware of an occurrence where this has happened in a customer environment. Only a specific combination of products and SFPs are impacted.
These HARDWARE products with SFPs (GLC-SX-MM-RGD and GLC-ZX-SM-RGD) are impacted:

Cisco will replace V02 (GLC-SX-MM-RGD and GLC-ZX-SM-RGD) parts that have 'ACW' at the start of the serial number only if it used on the switch product IDs (PIDs) referenced in the Problem Symptom section (CGS 2520, IE 3010, CGR-2010, GRWIC (CGS), IE2000, or IE2000U).
In order to replace the affected hardware, request an RMA through your normal support process.
Refer to the How to Identify Affected Products section for instructions on how to validate impacted units.

Method 1: Visually Inspect the Physical Product Label
For GLC-SX-MM-RGD, on the main serial number label on top of the unit, the serial number needs to begin with 'ACW'. The version ID (VID) is V02. The Cisco part number (CPN) is 10-2274-02.
The side labels have a CLEI of WDUIAACBAB with an ECI of 186657 on the scannable side label barcode.
Note: The serial number 'ACW' prefix needs to be verified along with one of the other attributes (V02, CPN, CLEI, or ECI).

For GLC-ZX-SM-RGD, on the main serial number label on top of the unit, the serial number needs to begin with 'ACW'. The VID is V02. The CPN is 10-2266-02.
The side labels have a CLEI of COUIAF5CAB with an ECI of 186659 on the scannable side label barcode.
Note: The serial number 'ACW' prefix needs to be verified along with one of the other attributes (V02, CPN, CLEI, or ECI).

Method 2: Find the Serial Number and Version Remotely from the Switch
Enter the show inventory command in order to obtain the serial number and version of the SFP. See this image:
The serial number date code range is between ACW1949xxxx and ACW2039xxxx for the GLC-SX-MM-RGD and ACW1947xxxx and ACW2032xxxx for the GLC-ZX-SM-RGD.
