A limited number of GLC-SX-MMD small form-factor pluggable (SFP) connectors might short-circuit when fully inserted into a switch port. The potentially affected units were produced between the weeks of 1 May 2017 and 4 June 2017.
The issue is the result of a manufacturing defect which affects a very small batch of components and results in a dimensional offset in the PCBA gold fingers.
The gold fingers of the pluggable SFP might short-circuit when the affected module is plugged into the switch port. Two possible scenarios could be encountered:

Damage might include a visibly charred or singed connector. The risk of thermal damage that spreads beyond the SFP connector and/or the mating connector is mitigated due to the use of flame retardant materials within the product.
Cisco recommends that you replace the GLC-SX-MMD SFPs when the previously mentioned symptoms are observed and the serial number matches using the Serial Number Validation Tool. Refer to the section "How to Identify Affected Products".
Note: There is no workaround for this issue.
Complete these steps in order to inspect the SFPs and avoid any potential damage to the system:
Caution: If the SFP is observed to have physical damage (item 3), do not attempt to reinstall the damaged SFP in any product. Quarantine the defective unit. Visually inspect the SFP cage and mating connector for any related damage. If damaged, an RMA should be requested for the individual linecard or for the complete stand-alone switch or router.
Complete these steps in order to determine the version and serial number of the product and verify whether the product is affected:
Identify the version and serial number of the affected GLC-SX-MMD. Use Method 1 or Method 2 in order to identify the version and serial numbers.
Method 1. Visually inspect the physical product label.
The serial number in the main top label needs to begin with 'FNS'. The version ID (VID) is V01. The Cisco part number (CPN) is 10-2626-01. The side labels have a Common Language Equipment Identifier (CLEI) of WOTRB9VBAA.
Note: The serial number prefix 'FNS' needs to be verified along with one of the other attributes (V01, CPN, or CLEI).

Method 2. Find the serial number and version remotely from the switch if the unit is already plugged into the port.
Enter the show inventory command in order to obtain the serial number and version of the SFP. Refer to this example:
